Until recently, reactive oxygen species, known as free radicals, were blamed for many cell-damaging processes. However, it is now known that highly reactive nitrogen compounds also exist in the body that can cause similar damage. Reactive nitrogen compounds include nitric oxide (NO) and its derivative peroxynitrite.

NO is formed in the body when oxygen reacts with L-arginine and is a relatively stable radical. In a further chemical reaction, NO reacts with other molecules to form the much more aggressive peroxynitrite. These reactive nitrogen compounds are responsible for a variety of important processes in the human organism as long as they occur in physiological concentrations. Their main function is to initiate programmed cell death. This ensures that certain damaged cells perish without affecting the neighboring tissue. NO is involved in bacterial defense and vasodilation. It acts as a neurotransmitter and controls functions in the mitochondria. Under certain conditions, the normally balanced nitrosative equilibrium, in which reactive nitrogen compounds are formed and then consumed, can get out of control.

Psychological and physical stress as well as external influences (e.g. infections, inflammation, medication intake, increased heavy metal contamination, vitamin deficiency) can lead to an uncontrolled proliferation of reactive nitrogen compounds and thus to the development of nitrosative stress.
